Important Forms for Student Athletes

Athletic teams are a fun part of Greenville Jr. High.  In order to try out for any sport, a student must have a signed athletic permission form on file for the year and a current physical.  Physicals are good for 13 months.  Both forms are available above.  Students must also keep their grades up in order to stay academically eligible for athletic events.  If you have any questions about your eligibility to try out for a sport, see Mrs. Loucks in the office.

Concussion Impact Testing

BCCU 2 and HSHS Medical Group have partnered to offer Concussion Impact Testing for 5th - 8th grade students.  High school students receive this test as part of the district concussion management plan.  The new service for 5th - 8th graders is optional.  If you would like to have your child tested, please fill out both of the forms on the right and return with $5 to the office.  Brandon Kircher, Athletic Trainer, will contact you to set up testing.  If you have any questions, you may contact the office at 664-1226.
